Please list below any Disabilities, Learning or Medical conditions, which you believe may affect your ability to take part in archery. (This information will be treated in absolute confidence and will only be used by your coaches to determine the most appropriate equipment and teaching styles to suit your personal needs.)
If the booking is for a junior, you confirm that you are their parent or guardian and consent to their participation in a training session with Sway Bowmen.
If the junior is under 16, you confirm that you will remain with them during the session.
1) Due to the nature of the sport, minor physical contact may be necessary as part of coaching.
2) The coaches have the authority to remove a participant from, or to suspend the session if safety instructions are ignored.
3) In the event of circumstances leading to the cancellation, Sway Bowmen’s liability is limited to a full refund of the fee.
4) The training is conducted in accordance with Archery GB rules of shooting by an affiliated club and supervised by experienced archers. It is therefore covered by the Archery GB public liability insurance arrangements; further details are available on request.

Note 1: The personal information gathered on this form will only be used to enable the coach/club to contact you if there is a change in the course timing or in any matter relevant to the course. This data will not be passed to any third party and will be destroyed on completion of the course.
